Work Boots Benefit
If you have a qualifying preventive care exam and receive SASMI’s Annual Physical Benefit, you may be eligible for a credit for a pair of USA-made, union-made, work boots as long as you haven’t received a credit for a pair within the three stabilization periods prior to applying.
If you qualify, you may select your pair of Thorogood or Carolina boots from the SASMI-approved work boots. You’ll be responsible for the full cost of any boot not on the approved list.
Applying for the Work Boots Benefit
- Receive an eligible routine preventive care exam from a qualified medical professional.
- Log in to the member portal and click on “Applications” and then “Annual Physical Benefit” to access the online form.
- Complete the online form.
- Upload a copy of your SMART-affiliated health and welfare fund’s Explanation of Benefits (EOB) that shows that you received the preventive care from a qualified professional.
- Provide your bank routing information for direct deposit. The Annual Physical Benefit will be deposited in your account.
- Once your claim is approved, if you are eligible for boots, you’ll be emailed instructions on how to apply for the Union Boot.
- From there, you’ll select your SASMI-approved Thorogood or Carolina boots.
- Shipping, returns, and exchanges are free. And there’s no sales tax (unless you live in Wisconsin).
The Work Boots Benefit does NOT reduce your future SASMI benefits.
If Your Claim for the Work Boots Benefit Is Denied
If your claim for the Work Boots benefit was denied, it may be because:
- you bought a pair too recently (you are eligible once every two years),
- you didn’t receive your Annual Physical Benefit from SASMI (which is a requirement to get your Work Boots), or
- you tried to purchase a pair of non-SASMI-approved work boots.
